Adult Dance Class Waiver & Release of Liability

Instructor / Organization: Jiaren Dance LLC

1. Acknowledgment of Risk
I understand that participation in dance classes, rehearsals, workshops, and related activities involves physical movement and carries inherent risks. These risks may include, but are not limited to, slips, falls, muscle strains, sprains, injuries, or other physical harm.
I voluntarily choose to participate and assume all risks associated with participation.
2. Release of Liability
In consideration for being permitted to participate, I hereby release and hold harmless the instructor, organization, property owner, and any affiliated parties from any and all claims, demands, causes of action, or liability arising out of or related to my participation, including claims of negligence to the fullest extent permitted by law.
3. Medical Responsibility
I certify that I am physically fit to participate. I understand that I am responsible for consulting a physician if I have any medical concerns.
In the event of an emergency, I authorize the instructor or staff to seek appropriate medical assistance if necessary.
4. Personal Property
I understand that the instructor and facility are not responsible for any lost, stolen, or damaged personal property.
5. Media Release 
I grant permission for photos or videos taken during class to be used for promotional or educational purposes.
6. Agreement
By registering for and participating in classes, I acknowledge that I have read and understood this waiver and voluntarily agree to its terms.
